Making the most of the Center for Health & Wellbeing starts with our Wellbeing Survey! The Survey is a self-guided tool that allows you to discover where you are in your present overall wellbeing and identifies the areas of improvement that are most critical to your ongoing health and happiness.
Facilitated in complimentary sessions with Sara Wright, the Center’s Wellbeing Guide with more than 20 years’ experience, the Wellbeing Survey provides a personal, curated approach to self-improvement with a focus on the Seven Dimensions of Wellbeing.

Your journey begins. During your appointment, you’ll take the CHWB Wellbeing Survey with the help of Sara. Sara will interpret your survey results, illuminating which dimensions of wellbeing are most important to your optimal health. You’ll spend time mapping out your personal journey to get there, based on recommendations around Center programs or services we encourage you to take advantage of.
Participate and stay engaged. Sara will provide regular, ongoing recommendations around Center-related programs or services, and suggestions from our community partners, that will play an important role in you getting to your wellbeing destination.
Check-in and re-assess. Six months after your initial appointment, Sara will invite you back for a re-assessment. These check-ins are critical to tracking your progress and realigning your journey, if needed.
